• Our business hours are 8:00 am to 5:00 pm, Monday through Friday. Please call for information on our extended hours.

Patients should bring the following with them to their appointment:

• Driver’s License or valid identification

•Insurance information

•Completed New Patient Packet (for patients new to the practice)

•Referral or authorization (if required by your insurance plan)

•X-rays, MRI, CT Scan images (usually on disc)

•Written reports of all test results, including lab, X-ray, MRI or any other imaging test pertaining to your visit

•List of medications you take including name, dosage, and frequency

•Copay – Cash, Check or Credit Card

If you are being seen as a result of a Motor Vehicle Accident, please bring the following with you to your appointment:

•Driver’s license or valid identification

•Completed Motor Vehicle New Patient Packet

•Auto Insurance Card (from car involved in the accident)

•Auto Insurance Declaration Page

•Police Report

•Claim Number

•Name of Adjuster

•X-rays, MRI, CT Scan images (usually on disc)

•Written reports of all tests, including lab, X-ray, MRI or any other imaging test pertaining to your visit

•List of medications you take including name, dosage, and frequency.

•Refills can be requested by phone or through our Patient Portal.

•Refill requests received after 3:00 p.m. cannot be processed until the next business day.

•Those prescriptions that cannot be called into your pharmacy can only be picked up by the patient or authorized individual. This individual must bring written authorization and valid identification.

•Prescriptions will only be called into the pharmacy we have on file for you.

We do not have a formal No-Show Policy; however, we do appreciate timely notification from you if you cannot keep your appointment so that we can accommodate other patients.

Seaview Orthopaedics is a participating provider with numerous insurance plans, including Aetna. Plan types and policy provisions vary greatly from one policy to another and, as the policyholder, it is crucial that you are fully aware of the benefits available under your plan. Before your appointment, please take the time to familiarize yourself with your insurance coverage and be sure to bring your insurance card to each visit. If you are a member of a managed care plan that requires a referral from your primary care physician (PCP), we must have this authorization prior to your appointment. Our precertification specialist will contact your insurance company for authorization and benefit information for diagnostic testing such as MRI or CT scans and for surgery. We will submit claim forms on your behalf but are bound by the provisions of your policy. If you have any questions about the requirements of your specific plan, please contact your insurance company. Third-party billing (such as liability or motor vehicle accidents) may require special consideration.
