There are two easy ways to log in to your portal.

When you book your first appointment, login credentials will be sent directly to your email address. Select “View your health record” and enter the username and password provided.

If you cannot locate your credentials, you can enter your phone number to recieve a one-time login code. Enter your first name, last name, and date of birth. Press submit and a one-time code will be sent to the number we have on file.
